With true disc or nerve root pain, other injection procedures may be considered. Lumbar and caudal "epidural steroid injections" promote the decrease in disc swelling and inflammation within the epidural space. A selective nerve root block may provide a more localized and specific block to the particular nerve root responsible for the radicular pain. These procedures are often done under fluoroscopic guidance to determine that the injection is placed in the appropriate location. It is virtually impossible to selectively inject the nerve root without using fluoroscopy. For this reason, epidural blocks are usually performed in an outpatient surgical setting and not in the office.
