FOREWARD

Firstly, I would like to express my thanks to all respondents who participate, and completion of the survey is entirely voluntary.

This survey will examine four important concepts of how women cope with endometriosis associated pain, and with "unexplained pelvic pain". These are their standards of care, how much support they have, their beliefs about their condition, and their experience of pain. Together with previous research, your responses are a distinctive contribution towards increasing knowledge. We hope that these findings will help address the needs of women in the future.

The focus of the survey is on your beliefs about your pain and your social support, and this obviously points to the need for you to be clear about your needs in this area.

All the information collected in this survey from all respondents will remain entirely confidential and will not be associated with you as an individual in the research paper. I ask you to study and consider your responses with honesty and speed so that results may be carried forward as a reflection of your experience. Thank you for your time and good luck to all concerned.
