Passports state very clearly that Medicare will NOT pay for "out-of-country" medical expenses!

I've been told some insurances have an " out-of-country" clause, which states the insurance company will consider "out-of-country" expenses for medical/surgical care.

If your insurance has approved "out-of-country" insurance coverage for you, what kind of documentation did you have to provide your insurance company with to gain their acceptance?
