A.) Patient Bill of Rights

1.) Patient has the right to safe, effective, ethical, and cost effective medical care.

2.) Patient has the right to understand tests, medications, procedures and treatments, their risks, their cost and their alternatives.

3.) Patient has the right to know the name of your health care providers, their roles and responsibilities.

4.) Patient has the right to have your privacy and personal dignity maintained.

5.) Patient has a right to expect that all communication and records pertaining to his/her care is treated as onfidential.

6.) Patient has the right to seek a second opinion.

7.) Patient has the right to an "Informed Consent", to be informed as to nature and purpose of technical procedures to be preformed.

8.) Patient has a right to communicate with those responsible for his/her care, to receive information regarding the nature and extent of the problem, to participate in the planned course fo treatment and prognosis.

9.) Patient has the right to be treated with respect and courtesy, to be addressed by your proper name. To be listened to when you have a problem or a question, and to receive an appropriate response.

10.) Patient has the right to see and obtain a copy of your medical record.

11.) Patient has the right to a detailed, itemized copy of your medical record.

12.) Patient has the right to refuse treatment and should be informed of the clinical consequences of that decision.

13.) Patient has the right to voice complaints.

14.) Patient has the right to have your family, religious or cultural preferences acknowledged and adhered to within the bounds of safe medical practice.
