A.) Patient Rights: As a patient at this clinic, you can expect:

1.) Your reports of pain will be believed,

2.) Information about pain and pain relief measures,

3.) A concerned staff committed to pain prevention and management,

4.) Health professionals who respond quickly to report of pain, and effective pain management

B.) Patient Responsibilities: As a patient in this clinic, we expect that you will:

1.) Ask your doctor or nurse what to expect regarding to pain and pain management

2.) Discuss pain relief options with your doctors and nurses,

3.) Work with your doctor and nurse to develop a pain management plan,

4.) Ask for pain relief when pain first begins,

5.) Help your doctor and nurse assess your pain,

6.) Tell your doctor or nurse if your pain is not relieved, and,

7.) Tell your doctor or nurse about any worries you have about taking pain medication
